dont operate on plantar wart: often recurs in scar tissue and very painful

manage with resus, IV Abx. major amputation if life threatening sepsis, debridemendt of skin and fascia,

• M. Ulcerans - daintree ulcer. incubates 4-7months. people probs infected in wet season and present later.
  • cutaneous infection initial
  • starts as skin nodule
  • ulcerates as subcut fat necroses
  • undermined ulcer with sloughy base, area of surrounding induration - active infection
  • painless unless secondary infection
  • may heal with scarring and satellite ulcers
  • uncommonly - spreading infection, cellulitic appearance
  • treat with hyperbaric O2, heat treatment
  • rifampicin and clarithromycin first line ABx, 6-8 week course
