Due to glomerular disease

pathophysiology

Damage to GBM → massive leakage of albumin +/- other proteins

Causes:

• Primary glomerular disease eg. minimal change disease
• Secondary to:
  • DM
  • Amyloidosis
  • (many other causes)

Characteristics

• Proteinuria >3.5g/day
• 24hr urinary protein collection to confirm
• PCR >300mg/mmol, ACR >250mg/mmol
• Hypoalbuminaemia
• Oedema
• Hyperlipidaemia (especially ↑ cholesterol)
• Liver goes ++ synthesizing lipoproteins (and clotting factors)
