Due to glomerular disease
pathophysiology
Damage to GBM → massive leakage of albumin +/- other proteins
Causes:
- Primary glomerular disease eg. minimal change disease
- Secondary to:
- DM
- Amyloidosis
- (many other causes)
Characteristics
- Proteinuria >3.5g/day
- 24hr urinary protein collection to confirm
- PCR >300mg/mmol, ACR >250mg/mmol
- Hypoalbuminaemia
- Oedema
- Hyperlipidaemia (especially ↑ cholesterol)
- Liver goes ++ synthesizing lipoproteins (and clotting factors)
