- Ring the destination to find an accepting doctor, make sure to get their name and their consultants name
- Differentiated illness to specific wards
- Undifferentiated/critically unwell pts to ED
- Complete IHT form
- Prepare discharge summary and relevant correspondence
- For private hospital also need hard copy page with provider number (discharge letter)
- Continue pt cares until departure
