Opening Statement:

In order to find out what STIs to test you for/To find out what could be causing your symptoms, I just have to ask you a few questions about your sexual history. Would that be okay?

Privacy/Confidentiality (<18)

Everything we talk about here is confidential, it doesn’t go outside the team/clinic. A parent is not able to call up and get information about you. Sometimes I have to break that confidentiality – if I am worried that you or someone around you is at risk of harm. If I have to break our confidentiality, I will do my best to let you know first

Any pain with peeing, (abnormal) discharge, bleeding or pain with sex, any lumps or bumps or itch?

Have you had an STI screen before? When was your last screen?

Have you ever tested positive to an STI before?

Partners

Are your sexual partners male, female, trans or a combination?

Do you have sex with men, women or both?

How many sexual partners have you had in the last 3 months?
