Intestinal microbial, epithelial & immune dysfunction → autoimmune inflammation on gut wall in a genetically susceptible host

Pathogenesis

exposure to antigens within intestines which would have given which normally generates inflammation then stopped by IL-10

  1. excess TNFalpha
  2. decreased IL10
  3. uncontrolled inflammation

Indeterminate colitis

colonic cancer in IBD within 8-10years

more common in UC pancolitis

anti TNFalpha therapy (remicade) → remission
