- neuro signs, headache → stroke
- hearing loss/tinnitus → Meniere’s, labyrinthitis
- sudden onset, triggered by head movement, no spontaneous nystagmus. → BPPV → Dix Hallpike and Eppley then home
- Gradual onset, constant, worse on head movement, spontaneous nystagmus → acute vestibular syndrome (HINTS)
- +ve head impulse = vestibular neuritis
- -ve head impulse or bidirect nystagmus or vertical skew = stroke

what kind of dizziness?
Time frame? Episodic, continuous, spontaneous, triggered
- Vertigo: false perception of motion
- Presyncope: about to faint/lightheaded
- disequilibrium: feeling unsteady- gait and imbalance

- seizure (partial epilepsy)
- hypoglycemia
- hyperventilation
- vertebro-basilar insufficiency due to atherosclerosis/OA
