non-dihydropyridines - diltiazem/verapamil (cardiac specific)
MOA
- Inhibit influx of Ca2+ via L-type VGCC, normally causing contraction
- Variety of hypotensive effects depending on the subclass and location of calcium channels inhibited
- Dihydropyridines act predominantly as vasodilators with little cardiac effects
- Non-dihydropyridines vasodilate but also inhibit AV node conduction and reduce cardiac contractility (reducing DAD/EAD to suppress premature ectopic beats)
Indication: HTN and angina
Contraindications: contraindicated in systolic HF, hypotension, severe angina
