“Transfusion checklist”

  1. Make sure you need to do it
  2. Collect pretransfusion specimen (no more than 72hrs before transfusion)
  3. The laboratory performs compatibility testing.
  4. Give blood components and monitor patient clinical status closely for the first 15 minutes of each pack and at least hourly throughout the transfusion. (esp. for dyspnoea, decreased O2 sats, fever and/or chills, rigors, rash or urticaria).
  5. Temperature, pulse, respirations, and blood pressure must be recorded: prior to transfusion, 15 minutes after commencement, and on completion of each pack.
